High INR results and RAT suggesting omitting does

Dear Colm

One of our GPs asked me for advice on the below matter:

re high INR results and RAT suggesting omitting doses but giving 7 days dosing and total dose... and what to record when next attends re recent dose.

Dear Tanya,
RAT will recommend omitting warfarin 2-3 days, then restart at new dose at approx 75-80% of previous dose. The new dose regimen will be completes for the full 7 days.
This is to allow for occasions when patient may not be able to return in exactly seven days, RAT gives the full weekly dose.

RAT allows for the dose reduction, you do not need to alter the weekly amount. Just enter the INR as normal.
