Medication and warfarin calculation

Are the recommendations calculated on the INR result alone, or do they take into consideration current medication dosages?

Medication doses are only a problem if they are increased, decreased, started or stopped. A stable dose even though the drug may interact with warfarin is not a problem.


Our practice does not use the drug list on RAT. We leave it to the prescriber at the point of issuing the new dose or drug to decide.
The repeat prescriptions can change frequently and updating the RAT drug list is laborious. It would be useful to have a link with the clinical system to have it automatically updated though.
Generally it is best to repeat an INR if a new drug is started or stopped (if it interacts with warfarin). The doctor should give this advice as he issues the script.

THE DRUG LIST IN RAT:
RAT does have modifying parameters if a new drug is started.
Try adding a drug (e.g. omeprazole) to the RAT drug list and use a start date within the last 7 days. Then enter an INR to see what happens...
