stop and restart

My coleuges and I are have difference of oppinion with regard to rat stoping and restarting warfrin. e.g when rat says to stop for 2 days and restart. They are used to altering the does to what the patient will take during the next week. where as I understood the dose listed was the equivalent weekly dose on which rat was suggesting to restart the patient on. your advice would be appreciated

Dear Andy,
You are right.
Omit 2 days warfarin then restart on the weekly dose outlined by RAT.
You will see that the new dose is 75-80% of the previous dose.
A dose is given for every day in case the patient does not return on the required review date.
