            <description><![CDATA[ Hi Colm, we have a young man who recently started on warfarin and appears to be very sensitive to it.   He is an extremely fit and active man and we wanted to ask can a high level of exercise have an impact on warfarin.  We haven't managed to get him within target, any alterations to warfarin caused his INR to rocket or plummet.  He is definitely complaint with medications.]]></description>

            <description><![CDATA[ Hi Colm<br />
Hope you are well.  I reeived an email from the Board recently to say that I had not recorded the correct figures on my qaurterly return (enhanced services claim form).  They want the figures for the number of patients registered in a quarter and not those seen.  I replied saying that I did not know how I was expected to give this information, split down to the different levels, ie those seen in the Practice by the Practice Nurse and those seen in the community by the District Nurse AND in some months/quarters patients may have used both services due to the fact that they may have had a spell in hospital and are unable to attend the treatment room immediately following discharge.<br />
<br />
Can you advise how you make the return and if you do not have Practice staff involved and are not claiming the higher level, could you suggest how I do this.<br />

            <description><![CDATA[ Hi Colm, we are getting a error message appearing when we try to look at one patient's details in particular. The message reads &quot;runtime error&quot; and doesn't allow us to go any further. It only appears to be affecting one patient atpresent. We had a similar problem a month ago when trying to do an audit with the same message appearing. You advised us to download the update again, which we did and it worked ok after this. Would appreciate your help.<br />

            <description><![CDATA[ We have a patient who has just started warfarin. they commenced warfarin on the 17th at 1mg per day and first test INR thursday 20th at 1.0.  Rat software read as if she had 3mg over the week, but she really took 3mg in 3 days.   Rat software gave her new dose of 5mg over the week when they were in on Thursday. but this was a reduction and her INR was still 1.0.  Have you got any guidance on new patients starting warfarin.]]></description>

Is it possible to back RAT onto a USB Pen Drive, once this is on the drive should we be fit to open it, I have had a look at it once it is on the pen, but it is not in a readable format, how if we had to, could we get our database on a PC again if there was something happened and we only had the pen drive.<br />

We are migrating to EMIS Web and having all new computers in September, please advise:-<br />
Do we download the RAT onto each pc as if we are new users?<br />
How do we retrieve all the previous patient information stored on the old version?<br />
We havent yet updated to I-Rat, should we do this on all pc prior to getting new computers?<br />
Is there an additional registration fee for I-Rat (I think we have already paid for the old version this year)<br />

            <description><![CDATA[ RAT runs as a separate entity to our patient database, therefore we are having to transcribe the results and recommendations into the patients notes to ensure it is contemporaneous. Is there a way of extracting / cut &amp; pasting the information we see on screen to a form that can be put in the notes.<br />
Also on that point is there a way of changing the format of the print out to suit our printers. We have selected A5 print but it seems to be missing half the data on the A5 page. Similarly with the script selection. We do not have the same size prescription paper in Ireland.  Thanks]]></description>

Just wondering if you could offer some advice on what we should do if a patient freely admits that they have missed a dose.  <br />
<br />
Is it better to adjust the weekly regime for the last week to demonstrate the missed dose or doses?  Or would this only be advisable if the missed dose had been weekly since their last review.<br />
<br />
Or would it be better to just not adjst the dosing regime and bring the patient back for review in a week and ensure that they take the medication as prescribed.<br />

            <description><![CDATA[ Any suggestions for using the RAT programme when patients are going for surgery?<br />
We are starting to help with the bridging of warfarin for patients who are having surgery. Most will stop their Warfarin 5 days before their surgery and may be treated with Clexane. They will restart their usual dose of Warfarin on the evening after their surgery and will have their INR done 4 days later by their GP, if they have been discharged. Is it best to enter in the 0mg doses for the days that patients are off their Warfarin and the doses they take post op or just to go on the RAT recommendations on Day 4 as if they had never been off their Warfarin?]]></description>

Also,it would be very handy to audit pts who are past their treatment end date  -I`m not familiar with RAT (used INR star at prev practice which had the ability to run of a list of such pts ) - will I have to do this manually or is there a way to this more easily?<br />
